Order Gift Certificate Participating Merchant Agreement

I understand that $.40 shall be deducted from each complete transaction (gift certificate) and that a 4% fee will
apply to the ending total of sales each month, fees may be subject to change with notice. (initial here)X______.

I agree to supply Ordergiftcertificate.com, with approximately 30 gift certificates or more, complete with serial numbers to be sent whenever a transaction is received. After 20 gift certificates have been sold, I understand that a request will be processed from ordergiftcertificate.com to my business for an additional 20 certificates to be shipped to P.O. BOX 974, Gainesville, VA 20155. Furthermore, I understand that I will receive a statement during the first week of every month with certificate numbers that have been sold to protect the said business from fraud. I understand that if nothing has been sold for that month, I will receive no statement. (initial here)X______.

I authorize Ordergiftcertificate to accept gift certificate payments on behalf of my business and understand that included with the monthly statement, complete with certificate numbers and amount of sales, I shall receive a check for the total amount of sales minus above stated fees.
